About Us
Eternals Foster Care is a Utah Department of Health and Human Services (DHHS) contracted agency dedicated to serving individuals with Intellectual Disabilities (ID), Related Conditions (RC), and Acquired Brain Injuries (ABI).
We believe that every person — regardless of ability — deserves to be included, celebrated, and supported in all facets of community life. Our team works closely with individuals, families, and care coordinators to deliver services that are tailored, respectful, and truly life-changing.
Our services are delivered in full compliance with the Centers for Medicare & Medicaid Services (CMS) Home and Community Based Services (HCBS) Settings Rule.

We'll schedule a no-obligation conversation with you, the individual, and/or their support team to understand their needs and goals.
Once a match is confirmed, we'll coordinate with DSPD and the care coordinator to complete the required documentation and begin services.
Our team is introduced, schedules are set, and we begin delivering person-centered care tailored to the individual's support plan.
We accept referrals from DSPD care coordinators, families, and self-referring individuals. We're here to help every step of the way.
